Following the recent outbreak of the COVID-19 (Coronavirus) and in line with our increased healthcare screening procedures, you will need to provide the following information before health clearance is given for your employment.
Please answer the following: Have you been outside of the UK within the last 14 days passing through or staying in any of the following countries – this includes holidays?
Mainland China
Thailand
Japan
Republic of Korea
Hong Kong
Taiwan
Singapore
Malaysia or Macau
Iran
Cambodia
Laos
Myanmar
Vietnam
Northern Italy; anywhere north of Pisa, Florence and Rimini

Advice to travellers returning from China and specified areas:
Travellers returning from Wuhan and Hubei Province, China. If you have returned from Wuhan or Hubei Province in the last 30 days:
You must stay indoors and avoid contact with other people for 14 days and call NHS 111 to tell them of your recent travel to the city
